The Accreditation process for Volume Imaging Professionals may require you to provide additional information at a later date to substantiate or complete your application. This information is NOT required at this time, but you must agree to provide any or all of the following upon our request. Any of the information you provide will remain confidential and used only to verify your qualifications for Accreditation.
•_ Samples of flyers, pricelists or marketing collateral previously used in the business.
•_ Personal data needed to conduct a Criminal Background Check (Full name, SS#, DL#, home address.)
•_ Proof of Business Insurance • Copy of State Sales Tax Permit (N/A in Oregon)
•_ Business References or Testimonials from at least 3 accounts.
